In Cagayan, Sarah is best remembered for being blindsided at the beginning of the merge, after she considered herself to have the most power as the swing vote, which caused fellow tribe mate Kass McQuillen to flip on the majority alliance, and vote her out instead.
Returning from Cagayan to Survivor: Game Changers with a vow to play a more assertive game, Sarah found herself able to use her skills to help navigate herself throughout the season, assuming a much more aggressive stance in the game, and allowing for herself to accomplish strong strategic moves. After acquiring and successfully playing both the Vote Steal and Legacy advantages, helping orchestrate an array of blindsides, and forging strong social connections with a plethora of fellow castaways, the jury voted to award Sarah the title of Sole Survivor.

Sarah was placed on the Aparri tribe of Brawns on Survivor: Cagayan. While in the woods, Sarah interrogated fellow police officer Tony Vlachos, asking if he was a cop. He lied to her, saying no, but Sarah did not believe this as she is a cop. After some time, Tony decided to tell the truth. They bonded and formed an alliance called Cops-R-Us. However, Tony lied once again to Sarah saying that Cliff Robinson and Lindsey Ogle were gunning for her. Tony and Sarah convinced Woo Hwang and Trish Hegarty to come into their alliance and throw the challenge to vote Cliff out, who would then be followed by Lindsey. Despite Sarah, Trish, and Woo attempting to throw the challenge, Cliff single-handedly saved himself by winning the challenge for them continuing their winning streak and did not have to attend a Tribal Council. Sarah then said in a confessional that rival tribe Luzon deserved to lose after their terrible performance. At the tribe swap, Sarah was left as the only remaining Brawn still on Aparri, as all her tribemates swapped to Solana.
Sarah was thrilled when Jeff Probst told her tribe that Cliff and Lindsey had both been eliminated. When the Aparri tribe lost the challenge, Tony (on the Solana tribe) yelled "Top 5", which made Sarah feel excluded. Then came the merge where Sarah was the swing vote for both tribes. At first Sarah wanted to stick with the new Aparri tribe but Kass McQuillen questioned her loyalty which made Sarah feel insecure. Tony tried to make Sarah swear to side back with his alliance which she couldn't. Sarah became the swing vote. Kass continued to argue with Sarah which Trish saw as an opportunity. Kass and Tasha Fox asked Sarah if she was willing to send home Trish or Jefra Bland, which Sarah refused. Sarah would rather target a strong male who could possibly have an idol such as Tony or LJ McKanas. Trish and Tony realized that Sarah was no longer on their side. Trish went to talk to Kass to convince her to go with her alliance's votes.
All of the new Aparri voted Jefra while all of the new Solana voted Sarah. Kass flipped on her alliance and voted for Sarah, sending her home as the first jury member and in 11th place. The new Solana happily applauded themselves after voting out Sarah.
At Final Tribal Council, Sarah congratulated the finalists. Sarah called out Tony for breaking their promise to not vote each other out. Sarah then asked Woo if she played the same game as Tony did, would he vote for her. Woo said if she did it without the idols, he would vote for her. Despite liking Woo and angered with Tony, Sarah voted for the latter to become the Sole Survivor, contributing to his victory in an 8-1 vote.

Sarah started on the Nuku tribe where she aligned with Cirie Fields. Because Nuku won the first two challenges, they did not go to tribal council. After the tribe switch occurred, she, along with fellow Nuku's Andrea Boehlke Ozzy Lusth, Zeke Smith, and Cirie, and Mana member Troyzan Robertson, were switched to Tavua which was the third tribe. Tavua dominated the next three challenges. Despite being in the majority, she aligned with Troyzan who was the outsider.
After Tavua was dissolved, she along with Zeke, Ozzy, Tai Trang, and Andrea were switched back to Nuku. They were joined by Sandra Diaz-Twine and Jeff Varner. Sarah lost her first immunity challenge on Day 16. At tribal council, Sarah joined the majority in voting out Sandra. Nuku lost immunity again on Day 18. At Tribal Council, Sarah joined the others in verbally voting out Jeff.
Sarah managed to reach the merge. She then joined an alliance led by Cirie and Andrea. Despite being apart of that alliance she would navigate between her alliance, and the rival alliance led by Sierra Dawn Thomas and Brad Culpepper.
At the Final 11, Sarah managed to retrieve the vote steal advantage. At the final 9, Sarah met her husband Wyatt at the Loved Ones Challenge. Though the team of Aubry, Andrea, and Brad won, Sarah, along with Cirie, were chosen to join them on the Reward Challenge along with their loved ones. Sierra told Sarah she has the Legacy Advantage and she will give it to her if she is voted out; so then Sarah orchestrated her elimination. Sierra ended up giving Sarah the legacy advantage, unaware that Sarah had blindsided her.
Prior to the final seven Tribal Council, Sarah gave her vote steal advantage to Cirie to build more trust. At Tribal Council, Cirie tried to play it, but Jeff disallowed her to do that because it was Sarah's. Sarah asked for the advantage back and Cirie gave it to her. She then played the advantage, putting two votes on Michaela Bradshaw. Brad and Troyzan voted for Michaela as well so she was blindsided. After returning from Tribal Council, Sarah confronted Cirie for using her own advantage and Cirie told her that she did that because Tai was coming after her. After finding out from Tai that he was going after her, Sarah apologized to Cirie and rebuked Tai for backstabbing her.
On Day 36, Brad won immunity and chose to share the reward with Sarah and Troyzan. On the reward, the three agreed to target Tai. At tribal council, Sarah played the legacy advantage which negated the three votes cast against her, Tai played an idol on himself and Aubry Bracco, Troyzan played an idol on himself, so this left Cirie as the only non immune person, so she was eliminated by default. On Day 37, Sarah and Tai were caught in the middle between the duo of Brad and Troyzan who were targeting Aubry, and Aubry who was targeting Tai. Both Sarah and Tai ended up siding with Brad and Troyzan so Aubry was unanimously voted out. On Day 38, Brad and Troyzan targeted Tai while Tai targeted Troyzan, so Sarah was the swing vote. Tai tried to persuade Sarah to enforce a tie, but at Tribal Council, Sarah sided with Brad and Troyzan so Tai was unanimously voted out. Sarah was now in the Final Three with Brad and Troyzan.
Sarah received mixed reception at the Final Tribal Council. She was lauded for her strategic control and social flexibility, but condemned for using social relationships for strategic gain and her numerous betrayals. Sarah's strategic control throughout out the post-merge phase beat out Brad's dominance in immunity challenges, and Troyzan's lack of gameplay and she was awarded the title of Sole Survivor in a 7-3-0 vote only losing Ozzy's, Debbie Wanner's, and Sierra's votes.
